Brutal Ball Manager 2 is a franchise management simulation based on American football in a fantasy setting, featuring 8 v 8 matches between teams of orcs, elves, dwarves and other races.

Brutal Ball Manager 2 is a franchise management simulation built around a fantasy version of American football. Matches are played 8 v 8, with each player taking part on both offence and defence. Teams are drawn from 24 fantasy races with more than 70 variants. Simulated matches use the same engine as those watched at normal speed, and every team in a league can be controlled by the user or handed to the AI at any point, which allows for hot seat and streaming multiplayer leagues. The game supports Steam Remote Play.

Franchise features include a draft, an off-season free agency system, in-season events, end of season awards, league records, career statistic tracking and a Hall of Fame. Players have personality traits that affect weekly events, free agency and on-field performance, and those with extreme personalities are given tags that hint at the benefits or problems they may bring to a team. Player ratings are broken down into dependent sub-traits, so a general passing rating is split into separate figures for short, medium and long passes, for example. Alongside injuries and deaths, players can now be knocked out of a match without suffering permanent stat loss, which places more emphasis on bench depth.

The sequel adds in-game replays and a streamer mode that automatically adjusts match speed depending on how close the score is and how important the fixture is. League management tools include exporting players as draft classes to other leagues, swapping franchises between leagues to build promotion and relegation systems, a schedule editor, a custom awards creator and an in-league tournament creator. Free agency runs across the full season through a weekly claim system tied to team appeal, and coaches can be hired or dismissed at any time, with mid-season sackings costing appeal. Substitutions are handled by assigning specific players as substitutes in the roster manager. The game includes a team and player editor, logo import, moddable game variables and editable history, coach and schedule files, along with a redesigned interface and a news ticker.
